It took us many, many hours to create this survey template. We collected surveys from numerous women’s ministry how-to books and online questionnaires. In addition, we used questions from women’s groups who had submitted their surveys to us. Finally, we compiled all of that information and designed a template that would incorporate every aspect of women’s ministry. We tried to include it all.

The short survey is two pages long, neatly fitting onto the front and back of one 8-1/2 x 11” sheet of paper. There is also an optional third page which provides more detail.
